• identification of the copyrighted work;
• identification and location of the material that you want deleted;
• your name, address, telephone number, and email address;

• a statement, made under penalty of perjury, that the notice is accurate and that you are the copyright owner or authorized to act for the owner.

Counter-notice

If your content was deleted because of a copyright notice and you believe that was a mistake, you may send a counter-notice that includes:

• identification of the deleted material and its former location;
• a statement under penalty of perjury that the deletion was a mistake or misidentification;
• your name, address, and telephone number; and
